RESOURCE COORDINATION

Resource coordination is an important extension of our street outreach services. Rather than a linkage model of care, we utilize a continuous care approach to ensure we are walking alongside and supporting the people we serve every step of the way. Through dedicated case management, we are able to assist with key resources such as obtaining a state ID, signing up for food stamps and medical insurance, providing transportation to and from appointments, and accompaniment throughout the entire process of navigating the shelter and housing systems-- just to name a few. All while listening, being guided by the individual’s own goals, and maintaining a humanistic and harm reduction approach.
